MEDICATION/TREATMENT ADMINISTRATION
In compliance with the Virginia Nurse Practice Act, we do not administer or supervise medication (prescription or over the counter medications) and/or medical treatment without a physician's order. In order to comply with SCPS regulations, we need a parent/guardian and physician signature for all medication/treatment administration. A new permission form must be completed and on file at the school annually for any medication/treatment. Most frequently used health care plans and the medication /treatment form are on the Health Services Forms Link at the top of this page.
Individual arrangements need to be made for transporting medication to and from the school. It is preferred that students do not transport medication to or from school. All medications must be presented in their original pharmacy container with the student's name, name of medication, name of prescribing physician, and dosage with beginning and ending date. This medication must be accompanied by physician's medication order and parental permission for administration.
All medications/supplies must be picked up by parents by the last day of school or they will be discarded by school personnel.
MEDICAL EXCLUSIONS FROM SCHOOL/REASONS TO STAY HOME FROM SCHOOL
- Nurse's judgment or parental discretion
- Fever over 100 orally within 24hr
- Active vomiting within 24hr
- Diarrhea within 24hr
- Swollen or painful joints from injury
- Severe earache
- Suspected contagious diseases (until on treatment for 24 - 48hrs)
- chicken pox
- conjunctivitis (pink eye) with drainage
- pediculosis (head lice)
- scabies
- undiagnosed rashes
- and other
- Injury or illness requiring further immediate treatment
- Suspected substance abuse
- Preschoolers: continual yellow or green drainage from nose

Rising 6th Grade Students

The Virginia General Assembly requires all 6th grade students to have a tetanus, diphtheria, pertussis (Tdap) booster shot prior to entry into the school this September, if at least 5 years has passed since the child’s last shot.
Please review your child’s shot record. No student will be admitted to a Stafford Middle School in September without this immunization. If their last shot was before 2001 please have this done prior to the first day of school. This shot may be listed as T, Td, Dtap and / or Tdap, call you doctor or local health department if you have questions.

ALL NEW KINDGARTEN STUDENT'S ENROLLING FOR 2009-2010 NEED TO HAVE A PHYSICAL EXAMINATION COMPLETED WITHIN A YEAR AND COMPLETED IMMUNIZATION RECORD COMPLETED AND SIGN OFF BY A PHYSICIAN.
